I have attached 2 order forms you may use either one to place your order.
One is a Microsoft Excel file that you can change the # of cards and the order will be calculated for you. Simply change the number of cards and print off the form.

Print off the PDF file and manually complete the order form. You do not need to calculate anything but the order total. I will send you the actual amount earned with your order.
** It is your responsibility to get the order forms to the office on time – Call me if you have an issue **

Express 2% Bonus Begins October 30
With the holidays approaching, Great Lakes Scrip Center and Express are pleased to be offering a 2% bonus effective Saturday, October 30, 2010, through Friday, November 19, 2010. That means Express $25 gift cards will have an 12% contribution instead of the normal 10%, during that period only. The Express Gift Card is redeemable for merchandise at all Express stores and on-line at express.com.
Hallmark 1% Bonus
Great Lakes Scrip Center and Hallmark are pleased to offer our customers a 1% bonus on Hallmark Gift Card purchases from Saturday, October 30, 2010, through Friday, November 26, 2010. That means our customers can purchase the $25 gift card with a 5% contribution instead of the normal 4% contribution.
CVS/pharmacy 2% Bonus Offer Begins October 30
Great Lakes Scrip Center and CVS/pharmacy are pleased to offer a 2% bonus contribution from Saturday, October 30, 2010, through Friday, November 19, 2010. This means that the CVS/pharmacy $25 and $100 gift cards, reloads and ScripNow! orders will all increase from a 6% contribution to an 8% contribution, during this time period only. The CVS/pharmacy card is redeemable at all locations nationwide for merchandise, including prescriptions.
Build-A-Bear Workshop 2% Bonus Begins October 30
Build-A-Bear Workshop® and Great Lakes Scrip Center are making your holiday shopping a little easier by offering a 2% bonus contribution effective Saturday, October 30, 2010, through Friday, December 3, 2010. The cards will have a 10% non-profit contribution, instead of the normal 8%, during that time period.
